A good friend of mine once asked me, "What is the point of off-menu dishes if non-regulars like them could not order them?"
I've recommended him many great dishes at a certain best restaurant last year and it turned out that he could not pre-order any - So I see his point.
Sometimes, off-menu dishes are work-in-progress in exchange for feedback from foodies and regulars before they are officially placed on the menu.
Other times, it's a special dish that is off-menu but you could still try to pre-order in advance if you know about its existence.
